I have a 2000 civic Si with a jdm b18c swap block only, I kept the b16a2 head. I have low compressor pistons and forged rods, can I use my stock ecu or do I need to tune and change the ecu setup?

Are you wanting to drive this thing for a long time or just move it around the driveway/shop?

Yes you can use same ecu but need to chip it.

you need to get it chipped for your setup

A real tune is the best, but I am sure xenocron or phearable have a tune that will get it running pretty damn good.

As others have said, get it tuned, but the b16 ecu is the ecu you'll want to run. I'd suggest getting all of the necessary bolt-ons you want, then tuning it. That way, you'll get the most out of your tune.
